• Print this form (in Microsoft Word) and mail with your check to make a memorial donation (in memory of a deceased person) or an honorary donation (to honor someone on a special occasion or for being a great person).

• An attractive notification card is sent to the person/family of your choice letting them know of your donation.  Donation amounts are not disclosed to the person.

• The ARC will not release your personal information to anyone.  However, we do print donor names to give recognition and thanks for contributions.  Please let us know if you do NOT want your name listed and we will list your donation as an “anonymous gift.”
